Evaluation of the outcomes of vagal nerve stimulation in children with drug‑refractory epilepsy in South Africa

For patients with epilepsy where seizures continue despite medications, vagal nerve stimulation (VNS) is a well-established alternative with reported efficacy rates of 60%. Although VNS has been studied, there have been few applications on a pediatric population in Sub-Saharan Africa, which the authors aimed to target. To fill this gap in research surrounding VNS, the authors collected data from 93 patients that were in the range of 6 months to 12 years old who all had drug-referactory epilepsy, meaning that anti-seizure medications were not effective. VNS implantation was performed by the same surgeon in all of the individuals who qualified based on unidentifiable lesions, divergent preoperative data, and underwent EEG and structural imaging. The researchers followed up over a 12 month period and found that there was a reported 50% reduction in seizure frequency from the patients baselines. The McHugh seizure reduction was used and it was found that there was an association between the length of follow ups and the McHugh seizure prediction score. Furthermore, there was found to be no meaningful reduction in the number of antiepileptic drugs following this procedure as only 3 patients from the sample were able to reduce the antiepileptic drugs that they used. The study had findings regarding VNS in pediatric Sub-Saharan populations, however, limitations include a small follow up period that may not account for the benefits of the procedure and a relatively small sample size. Through the increased use of VNS in clinical settings, the researchers conclude that physicians may favor a non-invasive method that contains few complications.

Vagal nerve stimulation (VNS) is important for the community as it has been found that the frequency of epileptic seizures can be reduced. If a patient has not responded to antiepileptic drugs, there are other methods such as VNS to try in order to improve their quality of life and reduce future seizures. This procedure can be applied to both adult and pediatric populations with promising results due to the low complication rates and the non-invasive nature of VNS. Through further studies, VNS could become more incorporated into clinical and caregiving settings to help patients suffering from epilepsy.
