Opthalmology

Comprehensive Summary

Drug development can be a long and tedious task across all fields of healthcare. The stages of research and development, performing clinical trials, and finally receiving FDA approval to distribute the drugs can take countless months and even years, leaving many desperate patients without access to novel therapies for prolonged amounts of times. This is where the intersection of AI and drug development comes into play. The article "Ophthalmic drug discovery and development using artificial intelligence and digital health technologies" by Cheng et. al provides a glimpse of how AI is absolutely revolutionizing modern drug development. The article outlines how AI is involved in the various stages of the drug development process including Target Identification, In-Vitro Stimulation, Pre-Clinical and Clinical Phases, Post-Market Drug Repurposing, and even its influence in sectors of Digital Health. One of the dominant strengths of AI is its ability to analyze and compile large multi-omics datasets of patient data, cross referencing it with real life 3 dimensional structures of molecules enabling AI to identify targets for drug molecules to attack. Furthermore, from a long-term patient health perspective, AI is being implemented into wearable devices like smartwatches and smartphones to monitor patient vitals over time and identify fluctuations to identify prime drug administration times. While the future seems very promising in the field of drug development, the article discusses multiple caveats to AI implementation including ethical concerns of having automated drug development which may be prone to error, may fail to take into account other patient health facets such as existing health conditions, or event fail to accurately represent certain patient demographics. The article ultimately stresses the importance of promoting transparency of artificial intelligence use in drug development and underlines the need for regulatory mechanisms to ensure it is performing to the highest standard for patient safety.

Outcomes and Implications

One of the biggest implications of artificial intelligence in medicine is its ability to streamline nearly all aspects of drug development, putting new and novel drugs out into the market more frequently, thereby drastically improving patient outcomes. Not only is the AI powered drug development process backed up by large datasets of data, but the intelligence also has access to extensive patient information, allowing for the creation and administration of specialized, patient specific drugs. This frontier has a lot of promise in the field of ophthalmology because it is very easy to access and image the eye, which provides a lot of fluid biomarkers allowing AI to scan the eye over time to notice developments in medical conditions for guided diagnostics and allowing it to synthesize drugs the target different areas of the eye, improving treatment optimization. Furthermore, ophthalmic repurposing is a huge area in drug development, and it refers to modifying existing drugs originally developed for non ophthalmic uses to treat various disorders of the eye. Generative AI can be a tremendous asset in this field due its ability to synthesize insights from already published studies and literature to identify practical uses of existing drugs to target disorders of the eye and drastically improve patient outcomes. This field has tremendous untapped potential, but ethical concerns are one of the biggest pitfalls of AI in medicine. In order to truly move forward in revolutionizing patient care and drug development, the use of AI must be made transparent and most of all equitable. This way, there can be a general consensus of trust in AI to provide accurate treatments and therapies for a very wide and diverse subset of patients.
